Aloha Symposium Friends,
We are excited to celebrate the 25th year of organizing and hosting our Big Island Annual Healthcare Symposium. This has been no easy feat for us over the years, but we continue to be passionate about doing our part to contribute to continuous learning and innovation in order to improve the health and well-being of our Hawai'i community.
In the three years since we held the Symposium in 2019, we have morphed and pivoted. Many of us restructured, merged and expanded. Some of us went out of business or decided to retire.
This Symposium continues to be the largest state-wide conference of its kind, organized by and for providers, with nationally acclaimed speakers who are experts vis-à-vis their respective topics. We also encourage Hawai'i audience participation for relevance to our own unique delivery system. In addition to our 'best of the best' plenary sessions, as in previous years, there will be face to face networking opportunities with provider colleagues and key Hawai'i healthcare influencers.

CME Credits
The American Academy of Family Physicians (AAFP) approved the conference for up to 11 prescribed 'live' continuing education credits (CME). Providers should claim only the credits commensurate with their attendance at each session.
CME credit request form is available online and at the registration desk. Certificates of completion will be issued shortly after the conference.
We kindly ask that participants complete the evaluation survey at the end of each session regardless of whether you are asking for continuing education credits. These surveys are required documentation for accreditation.
East Hawaii IPA dba Big Island Docs Continuing Education (CE) Disclosure Policy
It is the policy of the Big Island Docs dba East Hawaii IPA that all individuals in a position to control content disclose any relationships with commercial interests upon nomination/invitation of participation. Disclosure documents are reviewed for potential conflicts of interest and, if identified, are resolved prior to confirmation of participation. Only participants who have no conflict of interest or who agree to an identified resolution process prior to their participation were involved in the CE activity.

She will explore the evidence for pharmacological interventions with a focus on new GLP1 weight loss medications. Christopher Awtrey, MD, MBA, MHCDS
Associate Chief Medical Officer, Intermountain Health Medical Group Colorado
Christopher Awtrey, MD, MBA, MHCDS is Associate Chief Medical Officer for Intermountain Health Medical Group in Colorado where he is the physician lead for the over 500 Intermountain Medical Group employed providers between the Front Range and Western Slope. He was born and raised in Boston, Massachusetts. Dr. Awtrey is a Gynecologic Oncologist and served as Vice Chair for the Department of Obstetrics and Gynecology at Beth Israel Deaconess Medical Center (BIDMC) where he worked for 16 years before coming to Colorado. He also served as the Vice President for Network Operations and Provider Experience at Harvard Medical Faculty Physicians, which is the 1,900 member physicians’ organization for BIDMC. He attended Medical School at Hahnemann University in Philadelphia and completed his residency in Ob/Gyn at the University of Vermont, his gynecologic oncology fellowship at Memorial Sloan-Kettering Cancer Center in New York City, a Masters of Health Care Delivery Science at Dartmouth College and a Masters of Business Administration at Brandeis University. In his administrative capacity, he has an interest in approaches to alleviating physician burnout and improving the joy of practicing medicine.
Alan J. Bier, MD
President, Sharp Rees-Stealy Medical Group
Dr. Bier graduated Phi Beta Kappa from the State University of New York at Buffalo and attended medical school at the Albert Einstein College of Medicine, where he also completed his Residency in Internal Medicine and Fellowship in Cardiovascular Diseases.He is Board Certified in Internal Medicine and Cardiovascular Diseases. Dr. Bier is a Fellow with the American College of Physicians, American College of Cardiology and American Society of Echocardiography.He is a Fellow of the Council of Clinical Cardiology with the American Heart Association.Dr. Bier has been with Sharp Rees-Stealy in the Division of Cardiology since 1989 and served as the Chief, Division of Cardiology from 1995 to 2014.He was a longstanding member and Chairman of the Pharmacy & Therapeutics Committee from 1995 to 2013.This gave Dr. Bier the opportunity to represent Sharp Rees-Stealy on multiple committees for various health plans including Health Net, PacifiCare and the Sharp Health Plan.As a president, Dr. Bier continues to strive to maintain Sharp Rees-Stealy's position as a premier medical group.He currently also serves on the Board of Directors for Sharp HealthCare, the Sharp Health Plan, Continuous Quality Insurance and America's Physicians Groups
Tiffany Bierbrauer, BSBA, HM
Clinic Manager – Ambulatory Services, Virginia Mason Franciscan Health
Tiffany Bierbrauer is a healthcare operations expert at Virginia Mason Kirkland Regional Medical Center. She has a bachelor’s degree in Business Administration with HealthCare Management from Western Governors University.Tiffany's journey is defined by her dedication to optimizing operations and fostering team growth. Starting with her role as a certified medical assistant she cultivated hands-on expertise, laying the foundation for her current operational leadership role. Tiffany is a certified leader in VMPS and is spearheading innovation and optimizing workflows at Virginia Mason Kirkland. She focuses her work on process improvement, developing talent, and fostering team engagement with a wealth of experience and a forward-thinking approach.
Kenny J. Cole, MD, MS
System VP, Clinical Improvement, Ochsner Health
Dr. Cole began his role as System VP, Clinical Improvement for Ochsner Health in New Orleans, Louisiana in September 2019. He is a practicing primary care internist with advanced degrees from LSU Health Sciences Center and Dartmouth, as well as executive training from Harvard Business School. Prior to joining Ochsner Health, Dr. Cole was the Chief Clinical Transformation Officer for Baton Rouge General Medical Center, where he designed, developed, and implemented a completely reimagined multidisciplinary team-based model of primary care that focused on aligning clinical with financial outcomes. His current work at Ochsner Health built on that prior foundation to design, develop, and implement Ochsner 65 Plus, a group of redesigned primary care clinics focused on the unique needs of older adults. In addition, Dr. Cole serves as the Medical Director of Digital Medicine and Connected Health programs, which leverages digital technologies to enable remote patient management across a variety of chronic conditions.
Ian Duncan, PhD, FSA, MAAA
Chief Actuary, Arbital Health
Dr. Duncan is Chief Actuary of Arbital Health, a provider of software and services for value-based healthcare providers and payers. He founded Santa Barbara Actuaries Inc. (acquired by Arbital Health) and Solucia Inc. (acquired by SCIO Health Analytics) and is an Adjunct Professor of Actuarial Statistics at the University of California Santa Barbara. Dr. Duncan holds a graduate degree in economics from Balliol College, Oxford and a PhD in Statistics from Heriot-Watt University, Edinburgh, Scotland. He is a fellow of several actuarial societies. He is active in public policy and healthcare reform and served on the board of directors of the Commonwealth of Massachusetts Health Insurance Connector Authority from 2007-2014. Dr. Duncan currently serves on the board of a Medicare Advantage plan, as well as the board of the Society of Actuaries. He is the author of over 80 peer-reviewed papers on healthcare outcomes and predictive modeling, and several books. His most-recent book is a second edition of 'Healthcare Risk Adjustment and Predictive Modeling' (Actex Publications), published in May 2018.
Matthew Duncan, MD
Psychiatrist, Dartmouth Health
Dr. Duncan serves as the clinical director for Integrated Behavioral Health at Dartmouth Health (DH). He co-leads the team implementing the Collaborative Care Model (CoCM) for Dartmouth Health. For the past 14 years, He has provided direct, clinical consultation services within primary care and weekly case review as part of the CoCM model. DH team has successfully implemented an Integrated Care program based on CoCM, in fifteen of DH primary care clinics across the central and southern regions of New Hampshire. From 2017-2020, he served as a regional team lead for the state of New Hampshire's effort to expand integrated care across the state as part of the 1115 Medicaid Building Capacity for Transformation Waiver. In this role, he lead the team's efforts to advance their Collaborative Care program, not only within the Dartmouth Health system but across other primary care practices in the region, as well as building structured relationships with mental health providers and community partners who help address social determinants of health needs for patients. Dr. Duncan received his Medical Degree from Dartmouth Medical School where he completed his residency.
John Frownfelter, MD, FACP
Healthcare Industry Consulting Director – Lead for Data Driven Health, NTT DATA
Dr. Frownfelter brings more than 20 years of physician leadership experience to bear on the challenges in healthcare today. His HealthIT leadership roles in the c-suites of large provider organizations including Henry Ford Health, UnityPoint Health and Trinity Health, is augmented by more recent work in consulting and with healthcare startups. He has a broad range of expertise in organizational care transformation, change management with health IT, strategic planning and leveraging analytics to empower organizations at all levels.
Ana Fuentevilla, MD, MHCDS
CMO and Co-founder, Suvida Healthcare
Ana Fuentevilla, M.D., MHCDS, currently serves as Co-Founder & National Chief Medical Officer of Suvida Healthcare, a neighborhood-centric, senior-focused primary care business providing an exceptional consumer experience and a culturally relevant care model to underserved communities, beginning with the rapidly growing 65+ Hispanic population. Previously, she served as Optum's Chief Medical Officer, responsible for the overall clinical functions within Optum Population Health Solutions (PHS). Dr. Fuentevilla was responsible for the overall clinical functions within Community & State, including overall Quality and Affordability results and Model of Care programs. Before joining UnitedHealthcare, she served as the Director for the Division of Geriatrics and Long-Term Care at Maricopa Integrated Health System, Clinical Assistant Professor of Medicine at the University of Arizona, and staff physician at Tucson Long Term Care. Dr. Fuentevilla received her M.D. at the University of Arizona where she completed her residency in internal medicine. As a Flinn Foundation Scholar, she subsequently completed a fellowship in Geriatrics. She also received a Master in Health Care Delivery Science from Dartmouth College/Dartmouth Institute.
Richard Furlong, MD
Physician, Primary Care Section Head Medical Director, Virginia Mason Franciscan Health (VMFH)
Dr. Richard Furlong leads the acclaimed Kirkland Regional Medical Pavilion at Virginia Mason Franciscan Health which was profiled by the National Academy of Medicine as their index case study in burnout prevention, awarded the Board of Directors Award of excellence by ASHP, the largest U.S. clinical pharmacist society. He is a two-time winner of Virginia Mason's most prestigious honor, the Mary McClinton award, serves in leader roles for many clinical initiatives across VMFH primary care as well as in Washington State, serving as Co-Chair of the Washington State Medical Association's Workgroup on AI that will guide Washington State 2025 policy and legislation. His primary and enduring passion is building a structure for clinical teams to foster engagement, create paths of professional development, and reduce burnout. After completing degrees at University of Washington, Medical College of Wisconsin and an Internal Medicine Residency at Virginia Mason Hospital, he has worked at the Kirkland Regional clinic for 30 years.
Governor Josh Green, MD
Office of the Governor, Ke Keʻena O Ke Kiaʻāina; State of Hawaii
Governor Josh Green is a proud husband, father, and physician. For the past 20 years, Governor Green cared for Big Island families as a doctor. He served in the State House of Representatives and the State Senate from 2004 to 2018. As chair of the Senate Health and Human Services committee, he passed 'Luke's Law,' which provides care for children with Autism Spectrum Disorder, as well as tobacco and e-cigarette regulations, which increased the legal age for purchasing tobacco and e-cigarettes to 21. Passionate about housing solutions, Governor Green helped create 'Kauhale' communities to get unsheltered veterans, kūpuna and others into safe housing options. These efforts are ongoing and will continue to be one of his top priorities as Governor. Governor Green expanded the role of Lieutenant Governor by spearheading the largest healthcare response in state history during the COVID pandemic. These critical emergency efforts resulted in Hawaiʻi having the nation's lowest COVID-19 fatality rate. Governor Green received his BA from Swarthmore College and MD from Pennsylvania State University and completed residency at the University of Pittsburgh.
Joan Jeung, MD, MPH
Clinical Professor, Department of Pediatrics, Division of Developmental Medicine, University of California San Francisco (UCSF) School of Medicine
Joan Jeung MD MPH FAAP is a Clinical Professor in the Department of Pediatrics, Division of Developmental Medicine at the University of California San Francisco (UCSF) School of Medicine. Dr. Jeung's research interests center around innovative programs that strengthen behavioral and developmental healthcare capacity in pediatric primary care. Dr. Jeung serves as Senior Associate Director of the Child & Adolescent Mental Health Access Portal (Cal-MAP), the pediatric mental health care access portal for California. She also directs the Resilience Clinic at UCSF Benioff Children's Hospital Oakland, a primary care-based, caregiver/child group intervention for young children (ages 0-5) exposed to adversity. Outside of UCSF, Dr. Jeung serves on the Executive Committee for the newly formed American Academy of Pediatrics (AAP) Council on Healthy Mental and Emotional Development (COHMED) and on the AAP Pediatric Mental Healthcare Access National Advisory Group. From 2017-2021, she served on the inaugural executive committee for the Section on Minority Health Equity and Inclusion (SOMHEI).
Elizabeth Murphy, MD, DPhil
Professor of Medicine, Deborah Cowan Endowed Professorship, University of California, San Francisco (UCSF); Chief, Division of Endocrinology, San Francisco General Hospital
Dr. Elizabeth Murphy is Chief of the Endocrinology and Metabolism Division, Director of the Diabetes Center for High Risk Populations, and Director of the eReferral Program at Zuckerberg San Francisco General Hospital. She is also a Professor of Medicine at the University of California, San Francisco (UCSF) where she holds the Deborah Cowan Endowed Professorship in Endocrinology. Dr. Murphy’s work focuses on improving access and quality of endocrinology and diabetes care for low income, diverse populations across California through the development of novel care delivery models including the use of eConsult and the creation of novel multilingual low literacy teaching tools for diabetes care. She has worked with the Endocrine Society and the ACP on new models for better incorporating diabetes care into the medical home. Currently her team's efforts are focused on diabetes prevention in San Francisco. Dr. Murphy received her BA in Chemistry from Wellesley College, a DPhil in Biochemistry from Oxford University as a Rhodes Scholar, and an MD from Harvard Medical School in the Harvard-MIT affiliated Health Sciences and Technology program. She completed her internal medicine internship, residency and endocrinology fellowship training at UCSF. Previously Dr. Murphy served as Chief Medical Officer at KineMed Inc, a biotech company focused on the use of stable isotope technology for drug development.
Hilton Raethel, MPH, MHA
President and CEO, Healthcare Association of Hawaii
Mr. Raethel has more than 30 years of experience in the health care industry, including 12 years in Southern California and 20 years in Hawai`i. His areas of experience include business development, payor and providing contracting, medical and quality management, pay for performance, and legislative and regulatory advocacy. Before being named President and CEO at the Healthcare Association of Hawai`i in May 2017, Mr. Raethel served in executive roles for seven years at Hawaii Medical Services Association. Mr. Raethel also worked for ten years at Hawai`i Pacific Health. He has a BS In Psychology, a Master's in Public Health (MPH), and a Master's in Health Administration (MHA).
Barbara Spivak, MD
Immediate Past President, Massachusetts Medical Society; President, Mount Auburn IPA
Barbara S. Spivak, MD, has been a practicing internist at Mount Auburn Medical Associates for over 40 years with experience as both an owner and an employed ambulatory physician, as a hospitalist, and a physician leader in nursing homes. Since 1997, she has been the President and Chairperson of the Mount Auburn Cambridge Independent Practice Association (MACIPA).
Dr. Spivak is a vocal advocate for implementing payment reform, improving health care access, particularly to primary care, decreasing the administrative burdens on physicians, and addressing the inequities in health care. Since May 2021, she has been an officer in the Massachusetts Medical Society (MMS) and is its Immediate Past President. She is a long-standing member of the American Medical Association (AMA) and has served on the AMA's Integrated Physician Practice Council and the Organized Medical Staff Section. Dr. Spivak continuously works with the Massachusetts Health Quality Partners, the state's Quality Alliance Task Force, and CMS Technical Panels to evaluate and improve metrics for quality and payment. MACIPA has been a leading organization in the era of alternative payment models, being an early adopter of risk-based contracts, Medicare Advantage, and Medicare Shared Savings. MACIPA has used population health tools, care management programs, and quality improvement data to help physicians deliver the care patients need.

Raj Srivastava, MD, MPH, is the Chief Clinical Programs Officer at Intermountain Health, where he oversees nine system-wide Clinical Programs which align clinical providers from across the enterprise to deliver evidence-based best practice in the areas of behavioral health, cardiovascular, women's and neonatal health, surgical specialties and digestive health, acute and ambulatory medical specialties, musculoskeletal, oncology, neurosciences, and primary and preventive care. Dr. Srivastava also oversees Intermountain's Healthcare Delivery Institute (HDI), a center for education, implementation science, and applied health services research. The HDI has accountability for the spread of clinical evidence-based practices across the integrated healthcare delivery system. Dr. Srivastava is a tenured Professor of Pediatrics at the University of Utah in the Division of Pediatric Hospital Medicine and is a practicing hospitalist at Primary Children's Hospital in Salt Lake City. Dr. Srivastava trained at the Hospital for Sick Children in Toronto and at Children’s Hospital, Boston, Harvard Medical School. He is a past Chair and current advisory board member of Pediatric Research in Inpatient Settings (PRIS).
